Purpose: This study examined family health, resilience, and quality of life among primary family caregivers of individuals with mental disorders, and identified factors influencing their quality of life. Methods: A total of 130 primary caregivers from two psychiatric institutions in B Metropolitan City participated. Data were collected from October to December 2024 using structured questionnaires and analyzed with descriptive statistics, t-tests, one-way ANOVA with Scheffé post-hoc tests, Pearson's correlations, and multiple regression using SPSS 23.0. Results: Mean scores were 3.52 for family health, 2.71 for resilience, and 3.44 for quality of life. Quality of life significantly differed by caregiving burden and household income, and showed positive correlations with family health and resilience. Significant predictors were resilience (β=.62, p<.001), high caregiving burden (β=-.45, p<.001), moderate burden (β=-.26, p=.001), and monthly income over 4 million KRW (β=.18, p<.001). The model explained 70.3% of the variance. Conclusion: Resilience, caregiving burden, and household income were key determinants of caregivers' quality of life. These results underscore the need for nursing strategies that strengthen psychological resources, alleviate caregiving burden, and support economic stability.
